Born on the cob
It's certainly the softgoods; I can probably count on one hand the number of times I think softgoods have been implemented properly. But it's also the heads for me. It's not always the case, but I feel like more often than not they just look too bulbous. Maybe it's because they're dedicated sculpts designed to work on a basic, re-usable body, so they're quite literally not made for it, but so often they just look so big, even with the equally oversized softgoods. To a lesser extent, the hands as well- they so often just have the plain, kinda c-grip doll hands which I think really lack expression.
I think the only real exception to all that I can think of off the top of my head is the Captain Blake from The Fog. Because the softgoods are tattered, they don't look so tent-like, and his hands are uniquely sculpted. The head doesn't feel too too big either, even with the light-up eyes. I don't have him yet; haven't been able to find him for a decent price, but he's perhaps the only one so far I'd actually consider.
